Polycystic Ovary Syndrome and Obesity: A Lifespan Perspective

Polycystic Ovary Syndrome (PCOS) is the most common endocrine disorder in reproductive-age women, affecting approximately 10% of this population. In my clinical practice, I emphasize that PCOS is a chronic condition whose cardiometabolic implications persist across the lifespan, not just during the reproductive years. We frequently see a higher prevalence and severity among the Spanish, Native American, and Mexican descent communities we serve in our border region, as well as in other populations like South Asian women.

The endocrine and metabolic consequences—particularly insulin resistance, dyslipidemia, and cardiometabolic risk—extend well beyond the reproductive window, warranting proactive, longitudinal management (Teede et al., 2018; Azziz, 2018).

The PCOS–Obesity–Insulin Resistance Triad

I often ask patients to visualize a Venn diagram where obesity, insulin resistance, and PCOS overlap. Although insulin resistance is not a formal diagnostic criterion, it plays a central pathogenic role. Elevated insulin (hyperinsulinemia) enhances ovarian theca cell androgen production, amplifies LH pulsatility, and disrupts ovulation (Diamanti-Kandarakis & Dunaif, 2012). This drives a vicious cycle:

  • Hyperinsulinemia → Ovarian Androgen Synthesis → Anovulation: Elevated insulin directly stimulates the ovaries to produce more androgens (like testosterone), which interferes with regular ovulation.
  • Adipose Tissue Inflammation → Impaired Hormone Signaling: Fat tissue becomes inflamed, disrupting the function of hormones like leptin and adiponectin that regulate appetite and fat metabolism.
  • Insulin as a Fat-Storage Hormone → Increased Adiposity: High insulin levels promote fat storage and prevent its breakdown, reinforcing weight gain.

This triad explains why women with PCOS often experience menstrual irregularities, hyperandrogenism (acne, hirsutism), chronic low-grade inflammation, and an increased risk for type 2 diabetes, MASLD (metabolic dysfunction–associated steatotic liver disease), and cardiovascular disease.

To diagnose PCOS, we use the Rotterdam 2003 criteria, which require at least two of the following: hyperandrogenism, ovulatory dysfunction, and/or polycystic ovarian morphology on ultrasound, after excluding other causes (Rotterdam ESHRE/ASRM, 2004).

The Generational Impact of Maternal Health and Pre-Conception Care

One of the most compelling areas of modern medicine is epigenetics—the study of how our behaviors and environment can cause changes that affect the way our genes work. When we look at obesity, its epigenetic impact is profound and multi-generational. Research clearly shows that both maternal and paternal adiposity (excess body fat) significantly increase the risk of overweight, obesity, cardiovascular disease, and type 2 diabetes in their offspring.

This is why our work is so vital. By improving a woman’s weight and metabolic health before she conceives, we can make a substantial, positive impact on generations to come.

The Power of Pre-Conception Counseling and Weight Management

Given the high rates of obesity, pre-conception counseling is one of our most powerful tools. Lifestyle changes alone can reduce maternal weight gain by 3 to 9 kilograms (about 7 to 20 pounds) (Muktabhant et al., 2015). When combined with medications, the results can be even greater. The benefits are remarkable:

  • Lowered fasting insulin and glucose.
  • A 20% to 30% reduction in the rate of gestational diabetes (Chiefari et al., 2017).
  • A 20% to 40% reduction in macrosomia (a baby born significantly larger than average).

Our approach includes healthy nutrition, physical activity, and the strategic use of medications such as metformin or GLP-1 receptor agonists. For medications like semaglutide, it is critical to observe the recommended two-month washout period for both men and women before attempting conception.

Obesity and Pregnancy: Risks and Compassionate Care

Obesity in pregnancy intensifies insulin resistance and inflammation, increasing risks for both mother and baby, such as gestational diabetes, preeclampsia, miscarriage, and preterm birth (Catalano & Shankar, 2017). Once pregnant, the focus shifts to appropriate weight gain, with targets based on the mother’s pre-pregnancy BMI, always under the final guidance of her obstetrician.

Weight management extends far beyond diet and exercise. A person’s mental, emotional, and sleep health plays a pivotal role.

The Inflammatory Cascade of Chronic Stress

Chronic stress is a powerful driver of inflammation, a low-grade, simmering fire that disrupts nearly every system in the body. This inflammatory state is directly linked to weight gain. When the body is under constant stress, it elevates cortisol, promotes abdominal fat storage, and contributes to insulin resistance. It becomes a vicious cycle: stress causes inflammation, which leads to insulin resistance and weight gain, and the excess weight itself further fuels inflammation.

Navigating Psychiatric Conditions and Eating Disorders

Screening for psychiatric conditions is a standard part of my intake process. Conditions like depression, anxiety, PTSD, and bipolar disorder directly impact a patient’s weight journey. Many medications used to treat these conditions are “obesogenic,” meaning they can cause significant weight gain. My approach involves collaborating with the patient’s mental health provider to select medications that address both mood and weight, such as bupropion.

It is also vital to distinguish between a diagnosable eating disorder and disordered eating, which involves problematic behaviors that don’t meet full diagnostic criteria. Binge Eating Disorder (BED), the most common eating disorder, is characterized by recurrent episodes of eating large quantities of food with a feeling of loss of control. We can select targeted anti-obesity medications to address the underlying drivers of these behaviors, such as the bupropion-naltrexone combination for constant thoughts of food or GLP-1 agonists for cravings.

The Critical Role of Sleep in Weight Health

Sleep is not a luxury; it is a biological necessity. When it comes to weight, sleep is as critical as diet and exercise. Poor sleep disrupts our hunger hormones, suppressing leptin (the “stop” hormone) and boosting ghrelin (the “go” hormone). Sleep-deprived individuals tend to eat 300-500 extra calories per day and crave high-carb foods.

Obstructive Sleep Apnea (OSA), a condition where breathing repeatedly stops and starts, is a major concern. Excess weight is a risk factor for OSA, and the condition itself promotes further weight gain by disrupting hormones and increasing inflammation (Tasali & Ip, 2008). Screening for OSA is crucial, as managing weight without treating underlying sleep apnea is like fighting an uphill battle.

An Integrative Treatment Toolbox for Obesity

Evidence shows that even a modest 5–7% weight reduction can restore menstrual cyclicity and improve metabolic health in women with PCOS (Legro et al., 2013). Our multimodal approach is designed to achieve these outcomes safely and effectively.

Nutrition and Physical Activity

We prioritize eating patterns that reduce glycemic and insulin load, emphasizing protein with natural fats, high-fiber vegetables, and low-glycemic fruits. For physical activity, we focus on improving glucose disposal and insulin signaling. Short, frequent bouts of activity, such as three 10–15-minute walks per day, can be more effective for improving insulin sensitivity than a single long session (Hawley & Lessard, 2008).

Pharmacotherapy Under Medical Oversight

Under Dr. Cardenas’s medical direction, we use evidence-based pharmacotherapy:

  • Metformin: While off-label for non-diabetic PCOS, it is widely used to reduce insulin, regulate menses, and support ovulation (Teede et al., 2018).
  • GLP-1 Receptor Agonists: Medications such as semaglutide and tirzepatide achieve robust weight loss and improve insulin resistance, hepatic steatosis, and cardiometabolic risk (Wilding et al., 2021).
  • Endometrial Protection: For women with irregular menses, combined oral contraceptives can protect the endometrium. Spironolactone may be added to manage acne or hirsutism.

The Role of Integrative Chiropractic Care in Weight Management

A common barrier to successful weight loss is pain. When your back, knees, or hips hurt, the last thing you want to do is exercise. This is where integrative chiropractic care becomes a cornerstone of our treatment plans. My primary goal is to restore proper biomechanics and reduce musculoskeletal pain. Through spinal adjustments, soft tissue therapies, and corrective exercises, we can:

  • Alleviate Joint and Back Pain: Reducing mechanical stress on joints directly translates to less pain during movement.

Case Study 1: “Alex” – A Multifaceted Approach to PCOS and Obesity

“Alex,” a patient with PCOS, insulin resistance, prediabetes, and binge eating disorder, wanted to improve her health and lose weight. We started her on metformin and semaglutide, collaborated with her psychiatrist to switch her to a more weight-neutral antidepressant, and counseled her on a low-carbohydrate, high-protein nutrition plan. In just six months, she achieved a 15% reduction in total body weight, and all her metabolic lab markers normalized. Her case is a testament to an integrative strategy that treats the person, not just the number on the scale.

Case Study 2: “Natasha” – Breaking the Cycle of Generational Obesity

“Natasha,” a 33-year-old with class III obesity (BMI 40.9), prediabetes, and hypertension, wanted to prepare for a healthy second pregnancy. Her first was complicated by preeclampsia. We initiated a two-year plan that included metformin and tirzepatide, along with nutrition and activity counseling. Throughout this, regular chiropractic adjustments supported her nervous system and reduced physical stress.

After two years, her BMI dropped to 28.8, her prediabetes reversed (A1C from 6.1% to 5.5%), and her other health markers normalized. We then planned for pregnancy, discontinuing the tirzepatide with a proper washout period. Four months later, she conceived. She had an uneventful pregnancy and, most wonderfully, was successfully breastfeeding—a goal she was unable to achieve after her first child. Natasha’s story is a powerful testament to how addressing the root causes of metabolic dysfunction before pregnancy can transform a mother’s health and give her baby the best possible start in life.
