Understanding Hydroplasty for Frozen Shoulder

Hydroplasty is a procedure almost exclusively used to treat adhesive capsulitis. The core principle is straightforward yet profound: inject a large volume of fluid to mechanically stretch and break the restrictive adhesions within the shoulder joint capsule. This process, known as distension, creates space, reduces stiffness, and significantly improves range of motion, facilitating effective rehabilitation.

  • Superior is at the top, and Inferior is at the bottom.
  • Medial is towards the body’s midline, and Lateral is away from it.
  • You can see the layers of tissue: the outer deltoid muscle, the deeper infraspinatus muscle, and then the crucial posterior capsule of the glenohumeral joint.
  • Beneath the capsule lies the humeral head (the “ball” of the ball-and-socket joint).

The goal of hydroplasty is to precisely inject fluid into the space between the humeral head and the capsule, forcing the capsule to expand and release.

The Three-Step Procedural Protocol

The hydroplasty procedure is a meticulous, multi-step process designed for maximum efficacy and patient comfort. The technique combines a specific anesthetic, anti-inflammatory medication, and saline, all delivered under precise ultrasound guidance.

Step 1: The Suprascapular Nerve Block

Before addressing the joint capsule directly, the first step is to manage pain. The suprascapular nerve provides the majority of sensation to the shoulder joint. By performing a nerve block at the spinoglenoid notch—a small anatomical passageway just medial to the glenoid (the “socket”)—we can achieve significant short- and intermediate-term pain relief.

  • Why is this done? Numbing this nerve accomplishes two things:
    1. It makes the subsequent intra-articular injection far more comfortable for the patient.
    2. It provides a window of pain relief that allows the patient to engage more effectively in post-procedure rehabilitation without being limited by severe pain.
  • The Injection: Use a 5 mL syringe containing 1% lidocaine (a fast-acting anesthetic). The needle is guided via ultrasound to the spinoglenoid notch, and the anesthetic is delivered around the nerve.

Step 2: Numbing the Joint Capsule Pathway

Next, a local anesthetic is administered directly into the path the main injection will take. This desensitizes the deeper tissues and the joint capsule itself.

  • The Cocktail: A small amount of 1% lidocaine is mixed with sodium bicarbonate.

  • The Procedure: Using a thin, 25-gauge needle, the buffered lidocaine is injected into the posterior glenohumeral joint space, right under the capsule. Ultrasound imaging confirms the needle is in the correct location—above the humeral head and beneath the posterior capsule.

Step 3: The Therapeutic Distension

This is the main event. A large volume of fluid is injected to stretch and ultimately rupture the thickened, adhered joint capsule.

  • The Injectate: A 51 mL cocktail is prepared. Formulation includes:
    • 10 mL of 1% lidocaine: For immediate pain control.
    • 10 mL of 0.5% ropivacaine: A longer-acting anesthetic to provide extended pain relief post-procedure.
    • 30 mL of sterile saline: Provides the primary volume needed to distend the capsule.
    • 1 mL of Kenalog (40 mg): A potent corticosteroid to powerfully reduce inflammation within the joint, helping to prevent the adhesions from reforming.
  • The Procedure: An 18-gauge needle is guided into the same intra-articular space. Because of the large volume and pressure required, this is often a two-person job. The physician guides the needle, while a medical assistant pushes the fluid from the syringe.
  • The Visual Confirmation: Under ultrasound, the process is remarkable. As the fluid is injected, you can see the posterior capsule lifting away from the humeral head. The space visibly expands as it fills. Then, suddenly, you see the capsule deflate. This is the “break”—the moment the adhesions tear and the capsule is released. This visible deflation signals a successful procedure.
