Regenerative Medicine for Auto and Work Accident Joint Injuries

Abstract

Automobile crashes and work accidents can damage joints, tendons, ligaments, muscles, spinal discs, and nearby nerves. Pain medicine may help provide short-term comfort, but it does not always improve the injured tissue or correct movement problems that continue to stress it.

Regenerative and targeted injection treatments may offer another option for selected patients. Platelet-rich plasma (PRP), platelet-fibrin products (PFP), and microfragmented adipose tissue (MFAT) use materials taken from the patient’s own body to support healing. Epidural spinal injections work differently because they mainly reduce inflammation around irritated spinal nerves.

Peptide therapies are also being studied, but they require caution because human research and long-term safety information remain limited. A complete treatment plan may combine medical oversight, integrative chiropractic care, functional medicine, rehabilitation, and a gradual return to work or daily activity.

Why Joint Pain Can Continue After an Accident

A car crash, fall, lifting injury, or repeated work strain can affect more than one part of the body. A knee may twist, a shoulder may be pulled, or the spine may be compressed. The injury may involve:

  • Joint cartilage
  • The joint capsule
  • Tendons and ligaments
  • Muscles and connective tissue
  • Spinal discs
  • Spinal nerve roots

Swelling and muscle guarding may then limit movement. The body often protects the painful area by changing how it moves. A person with knee pain may limp. Someone with a shoulder injury may use the neck and upper back to lift the arm. A worker with lower back pain may avoid bending and place more pressure on the hips.

Over time, these changes may create weakness, stiffness, poor balance, and added strain on nearby joints. Treatment should therefore look beyond the pain score. The care team should identify the injured tissue, check for nerve irritation, study how the patient moves, and determine whether the joint is stable enough for daily activity and work.

PRP: Concentrated Healing Signals From the Blood

Platelet-rich plasma treatment begins with a blood draw. The blood is placed in a centrifuge, which separates and concentrates the platelets. The prepared PRP is then injected into a selected joint, tendon, ligament, or muscle. Ultrasound or another imaging method may be used to guide the injection.

Platelets are best known for helping the blood clot. However, they also release growth factors and chemical signals involved in tissue repair. These signals may help manage inflammation, support collagen production, improve blood flow, and create a better local healing response (UPMC, n.d.).

PRP may be considered for:

  • Knee, hip, shoulder, or ankle joint pain
  • Tendon injuries
  • Ligament sprains
  • Partial soft-tissue tears
  • Early or moderate osteoarthritis
  • Pain that continues after conservative care

PRP has more human research than many other regenerative treatments. A meta-analysis of randomized trials found meaningful improvements in pain and function for many patients with knee osteoarthritis. However, results may depend on the condition, platelet concentration, preparation method, number of injections, and the patient’s overall health (Bensa et al., 2025).

PRP should not be described as a guaranteed cure or as a method that can rebuild a completely damaged joint. It is better understood as a treatment that may improve the healing environment around selected injuries. Temporary soreness may occur, and improvement often develops over several weeks or months rather than overnight.

PFP: A Fibrin Framework for Repair

Platelet-fibrin products are sometimes called platelet-rich fibrin (PRF). Like PRP, they are made from the patient’s own blood. The main difference is the fibrin framework.

Fibrin is a natural protein involved in clot formation. It may create a soft scaffold that holds platelets, white blood cells, and healing signals near the treatment area. This framework may allow some growth factors to be released over a longer period than with certain liquid PRP preparations (Farshidfar et al., 2025).

Possible goals of PFP treatment include:

  • Supporting slow-healing soft tissue
  • Holding platelets near the injured area
  • Providing a temporary repair framework
  • Supporting tendons and ligaments
  • Working alongside controlled rehabilitation

PFP products are not all prepared in the same way. The research is also less standardized than the evidence for PRP. Patients should ask which platelet-fibrin product is being used, how it is prepared, why it is being recommended, and what research supports that specific use.

MFAT: Support From the Patient’s Own Fat

Microfragmented adipose tissue is prepared from a small amount of the patient’s own fat. The fat is collected through a minimally invasive procedure, washed, and mechanically processed into smaller sections.

MFAT keeps much of the fat tissue’s natural support structure. Adipose tissue contains signaling cells, blood-vessel support cells, and other components that may help manage inflammation and support tissue repair.

MFAT has been studied most often for joint conditions such as knee osteoarthritis. Research comparing MFAT with PRP has found that both treatments may improve pain and function in selected patients. However, MFAT is not automatically better than PRP, and the right choice depends on the diagnosis and the severity of the damage.

MFAT may be considered when:

  • Joint pain is more advanced
  • Cartilage wear is present
  • Earlier conservative care has not provided enough relief
  • The patient wants to delay joint surgery
  • A larger tissue-based product may be appropriate

MFAT is more involved than a blood draw. It requires fat collection, sterile processing, and careful placement into the treatment area. It also cannot guarantee that damaged cartilage will completely regrow. A medical evaluation is needed to review infection risk, bleeding risk, medications, medical conditions, and whether surgery may be the safer choice.

Academic medical centers commonly include MFAT as one possible part of an evidence-based regenerative medicine program rather than as a stand-alone cure for every joint problem (University of Iowa Health Care, n.d.).

Epidural Spinal Injections Calm Inflamed Nerves

Epidural spinal injections are sometimes grouped with regenerative treatments, but they are not regenerative in the same way as PRP, PFP, or MFAT.

Most epidural injections place a corticosteroid, sometimes with a local anesthetic, into the space around the spinal nerves. The medication is intended to reduce inflammation around an irritated or compressed nerve root.

Epidural spinal injections may be considered when an automobile or work-accident causes:

  • A herniated or bulging spinal disc
  • Sciatica
  • Cervical radiculopathy
  • Lumbar radiculopathy
  • Pain traveling into an arm or leg
  • Numbness or tingling caused by nerve irritation

The goal is to create a period of lower pain and better movement. This treatment window may help the patient participate more comfortably in chiropractic care, walking, physical rehabilitation, and strengthening exercises.

The benefits are often short-term. A 2025 American Academy of Neurology review found that epidural steroid injections may provide modest short-term pain or disability improvement for some people with radiculopathy. Long-term benefits were less certain (American Academy of Neurology, 2025).

Possible risks include:

  • Temporary blood sugar changes
  • Bleeding
  • Infection
  • Headache
  • Temporary numbness
  • Reactions to the medication
  • Rare nerve injury

Careful patient selection, sterile technique, and image guidance are important.

Peptide Therapies: Promising but Still Emerging

Peptides are short chains of amino acids that act as signals in the body. Some clinics discuss peptides such as BPC-157 or thymosin-related products for tendon, ligament, muscle, or joint recovery.

Animal and laboratory studies have raised interest in their possible effects on inflammation, blood-vessel growth, and tissue repair. However, strong human evidence remains limited. Large randomized clinical trials are lacking for many peptides promoted for musculoskeletal injuries (Levo Health, 2025).

The U.S. Food and Drug Administration has identified important concerns involving compounded BPC-157 and several other peptides. These concerns include possible immune reactions, peptide-related impurities, product-quality problems, and limited safety information. The FDA states that it lacks enough information to know whether compounded BPC-157 may cause harm when given to humans.

Peptide therapy should not be presented as equal to PRP or as a proven treatment for accident-related joint damage. A medical discussion should cover:

  • FDA approval and regulatory status
  • The quality of available human studies
  • Known and unknown risks
  • The source of the product
  • Possible medication interactions
  • Medical conditions that may increase risk
  • Better-studied treatment choices

Peptide therapy should never replace a correct diagnosis, rehabilitation, or necessary surgery.

How Integrative Chiropractic Care Fits In

An injection may support the tissue or reduce inflammation, but the patient still has to move, work, lift, walk, and regain strength. Integrative chiropractic care addresses the mechanical side of recovery.

Depending on the diagnosis and stage of healing, chiropractic care may include:

  • Gentle joint mobilization
  • Spinal manipulation when appropriate
  • Soft-tissue treatment
  • Posture education
  • Safe lifting instruction
  • Corrective exercises
  • Balance and walking training
  • Spinal decompression for selected conditions
  • A gradual return-to-work plan

The goal is not simply to “put a joint back in place.” A better goal is to improve safe movement, reduce stiffness, decrease repeated stress, and help the patient use the healing tissue correctly.

Recent clinical guidelines for lower back pain commonly include exercise, remaining active, and spinal manipulation among the non-drug options that may be considered for selected patients (Zhou et al., 2024).

Rehabilitation is also important because muscles protect joints by controlling movement. A combined care plan may follow these steps:

  1. Reduce severe pain and inflammation.
  2. Restore comfortable joint and spinal motion.
  3. Rebuild strength, balance, and endurance.
  4. Practice work and daily-life movements.
  5. Return to normal activity in safe stages.

PRP, PFP, and MFAT may support healing for selected joint, tendon, and ligament injuries after automobile or work accidents. Epidural spinal injections may reduce inflammation around irritated spinal nerves, but they are mainly pain and inflammation treatments rather than regenerative procedures.
