Frozen Shoulder Treatment With Hydroplasty Recovery Guide
Table of Contents
This educational post explores hydroplasty, also known as hydrodistension, a highly effective, minimally invasive procedure for managing adhesive capsulitis, or frozen shoulder. From my perspective as an integrative practitioner, I will guide you through the physiological rationale behind this technique and detail the step-by-step process as demonstrated by leading experts. We will examine the anatomical targets, the specific medications used, and how ultrasound guidance ensures precision and safety.
I will also explain how we incorporate this advanced procedure into a multidisciplinary framework at our clinic, where integrative chiropractic care plays a crucial role in post-procedural rehabilitation and long-term recovery. This article aims to provide a comprehensive understanding of hydroplasty, from the initial nerve block to the therapeutic capsular distension, and its place in a modern, evidence-based treatment plan for restoring shoulder function.
As a practitioner with a diverse background in chiropractic, functional medicine, and advanced practice nursing, I am constantly seeking the most effective, evidence-based solutions for my patients. At Injury Medical Clinic PA, my mission is to integrate the best of multiple disciplines to create a synergistic healing environment. A cornerstone of this approach is our multidisciplinary team, where I work alongside our Medical Director and Collaborative Physician, Dr. Maria Guadalupe Cardenas, MD. With over 40 years of experience as a board-certified internist, Dr. Cardenas provides essential medical oversight, allowing us to offer a comprehensive spectrum of care that bridges traditional medicine, chiropractic, and rehabilitative therapies.
Our collaborative model is particularly vital when addressing complex conditions like adhesive capsulitis, commonly known as frozen shoulder. This painful and debilitating condition involves the thickening and tightening of the glenohumeral joint capsule, severely restricting movement. While chiropractic adjustments and physical therapy are foundational to our approach, some cases require a more direct intervention to break the cycle of inflammation and adhesion. Today, I want to share insights into one such advanced procedure: hydroplasty, or hydrodistension.
I’ll present the latest findings and techniques from leading researchers in the field, showcasing their work to show how modern, evidence-based methods are transforming patient outcomes. We will look at a procedure performed by Dr. Paul Taylor, a primary care sports medicine physician with OrthoVirginia, to understand the intricacies of this powerful intervention.
Hydroplasty is a procedure almost exclusively used to treat adhesive capsulitis. The core principle is straightforward yet profound: inject a large volume of fluid to mechanically stretch and break the restrictive adhesions within the shoulder joint capsule. This process, known as distension, creates space, reduces stiffness, and significantly improves range of motion, facilitating effective rehabilitation.
To visualize what we’re targeting, look at this ultrasound image of the posterior (back) shoulder.
The goal of hydroplasty is to precisely inject fluid into the space between the humeral head and the capsule, forcing the capsule to expand and release.
The hydroplasty procedure is a meticulous, multi-step process designed for maximum efficacy and patient comfort. The technique combines a specific anesthetic, anti-inflammatory medication, and saline, all delivered under precise ultrasound guidance.
Before addressing the joint capsule directly, the first step is to manage pain. The suprascapular nerve provides the majority of sensation to the shoulder joint. By performing a nerve block at the spinoglenoid notch—a small anatomical passageway just medial to the glenoid (the “socket”)—we can achieve significant short- and intermediate-term pain relief.
Next, a local anesthetic is administered directly into the path the main injection will take. This desensitizes the deeper tissues and the joint capsule itself.
This is the main event. A large volume of fluid is injected to stretch and ultimately rupture the thickened, adhered joint capsule.
Hydroplasty is a powerful “reset” for frozen shoulder, but it is not the end of treatment. In fact, it’s the beginning of a critical window for recovery. This is where our integrative model at Injury Medical Clinic PA truly shines.
Following a successful hydroplasty, the patient has a unique opportunity. The adhesions have been broken, the corticosteroid is controlling inflammation, and the long-acting anesthetic is managing pain. Now is the time for active rehabilitation to restore function and prevent the scar tissue from returning.
Our approach, overseen by Dr. Cardenas and led by my chiropractic and rehabilitation team, includes:
This combined approach ensures that we capitalize on the mechanical benefits of hydroplasty. The procedure creates the opportunity for movement, and our integrated chiropractic and rehabilitative care teaches the body how to use that movement correctly and sustainably. By managing the entire patient—from the joint capsule to the spine to their overall inflammatory status—we can achieve lasting relief and a full return to function.
